What’s Included

Primary Care annual evaluation and labs managed by Oasis Healthcare NPs and PAs

Walk-In visits for acute illnesses and injuries during normal clinic hours 

Common labs (flu, strep, mono, CBC, PAP, urinalysis, cholesterol screening, etc)

Procedures including lacerations and wounds, sexual health, cerumen impaction, etc. 

What is the Cost

$149/month per person OR

$5999 annual family maximum

1 year commitment from sign on date paid once or monthly automatic withdrawal

Additional Fees

Specialty labs - including but not limited to: hormone testing, cultures, non standard vitamin levels, PCR covid tests for travel, etc. Can be billed at clinic negotiated rates or through commercial insurance

IV hydration services greater than 2 occurrences/year will be billed at listed prices.

Radiology - can be billed through commercial insurance or rates established at imaging centers.

Important Facts

*This is not insurance. This is a monthly or annual fee for access to Oasis Healthcare, LLC.  Any services provided through any other healthcare provider, organization, or entity are not covered in this agreement.

*Oasis Healthcare accepts several MEDICAID insurers. If you have MEDICAID, please contact the clinic directly before subscribing.

*If you plan to use HSA funds, please check with your financial advisor or HSA plan to be sure these can be used.

*Fees may be paid by a third party (employer, family member). Please note, if the subscribed individual is over the age of 18, an employee, and in other cases of legal rights to privacy, no additional information regarding visit types, visit frequency, cost will be provided to the payor. 

*After hours: We will check messages each morning at 8 am. If there is an emergency or urgent need that cannot wait, patients will need to seek emergency care.
